Eligibility is discussed when a patient’s physician determines that hospice care is appropriate. A team from Guardian Hospice will meet with the patient and family members to further discuss the patient’s ongoing care. In order to meet Medicare guidelines, the patient’s physician and the Guardian Hospice medical director must agree that the patient has a life-limiting illness that will no longer be aggressively treated. Hospice care can be a welcome alternative to ongoing painful treatments, procedures, and repeat visits to emergency rooms.
If the patient or surrogate decision maker agrees, hospice care will focus on comfort care instead of treatment designed to bring about a cure.
